I/We, the undersigned parent(s), or legal guardian(s) of the minor child whose name appears below, understand that dance, exercise or movement class requires physical exertion. I/We also realize that participation in dance classes and activities involves inherent risks, including but not limited to the risk of physical injury, and despite precautions, accidents and injuries may occur. By signing this Waiver and Release form, I/we assume all risks relating to our child's participation in dance classes and activities conducted by Liz Vacco and Petite Feet LLC (the "activities") and related to the use of any and all spaces (the "premises") used by Liz Vacco for the purposes of such activities.
I/We agree to release and hold harmless and covenant not to sue Liz Vacco, Petite Feet Dance LLC, its instructors, Athletic Garage Dance Center, G-Son Studios, Studio A, and any other owners or operators of the premises, and any of their officers, directors, independent contractors, employees and agents and representatives (each an "Indemnified Party") from any and all causes of action, claims, or demands now and in the future. I/We will not hold any Indemnified Party liable for any personal injury to our child or any property damage to our personal property, which may occur on the premises before, during or after our child's participation in the activities.
I/We acknowledge the contagious nature of COVID-19 and other contagious diseases and viruses and voluntarily assume the risk that I/we and/or my children may be exposed to or infected by COVID-19 by participating and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I/We understand that the risk of becoming exposed to or infected by COVID-19 and other contagious diseases and viruses may result from the actions, omissions, or negligence of myself and others, including, but not limited to, employees, independent contractors, volunteers, and program participants and their families. I/We release Petite Feet Dance LLC, its instructors, independent contractors and all associates from liability for harm, injury or death pertaining to COVID-19 and other contagious diseases and viruses.
I/We represent and warrant that I/we have read this Waiver and Release prior to its execution and fully understand its contents. I/We further represent and warrant that I/we am/are of legal age, am/are the parent(s) / legal guardian(s) of the minor child under 18 years of age who will be participating in the activities and whose name appears below, and have full power and authority to enter into this Waiver and Release. In addition, I/we agree that this Waiver and Release shall be binding on my/our heirs, successors, executors, administrators and assigns, and those of the minor child whose name appears below.
I/We, the undersigned parent(s), or legal guardian(s) of the minor child whose name appears below, grant to Liz Vacco and Petite Feet Dance LLC the right to take photographs or videos of my child in connection with Petite Feet Dance classes. The photographs and videos may be used for internal purposes, such as recording and practicing dances and for development of curriculum.
I/We agree that Liz Vacco and Petite Feet Dance LLC may use such photographs and videos of my child with or without my name and for any lawful purpose, including, for example, such purposes as publicity, illustration, advertising, and Web content. I/We authorize Liz Vacco and Petite Feet Dance LLC, its assigns and transferees to copyright, use and publish the same in print and/or electronically.
